Before submitting this application, you must read and agree to the following:

I ACKNOWLEDGE THAT ALL THE INFORMATION ON THIS FORM IS TRUE AND CORRECT. I UNDERSTAND THAT ANY MISREPRESENTATION OF FACT MAY RESULT IN THE REMOVAL OF THE FOSTER DOG FROM MY HOME BY THE KING GEORGE ANIMAL RESCUE LEAGUE.

I FURTHER ACKNOWLEDGE THAT AT ANYTIME, IF I CAN NOT OR DO NOT WANT TO CONTINUE FOSTERING THE DOG, I WILL RETURN IT TO THE KING GEORGE ANIMAL RESCUE LEAGUE ONLY, AND NOT TO ANY OTHER PLACE.

I UNDERSTAND THAT ANY ANIMAL FOSTERED BY ME/US FOR THE KING GEORGE ANIMAL RESCUE LEAGUE BELONGS TO THE RESCUE LEAGUE AND I/WE MUST TREAT IT AS SUCH.

I UNDERSTAND THAT ALL ADOPTIONS MUST BE HANDLED THROUGH THE KING GEORGE ANIMAL RESCUE LEAGUE, ALL DECISIONS REGARDING HEALTH/WELFARE OF THESE FOSTER ANIMALS WILL BE STRICTLY THE DECISION OF THE KING GEORGE ANIMAL RESCUE LEAGUE BOARD.

IF A FOSTER HOME WISHES TO ADOPT THEIR FOSTERED PET, THEY MUST SUBMIT AN APPLICATION UNDER THE SAME GUIDELINES AS OTHER ADOPTERS.

AT ANY TIME, IF CONDITIONS ARE NOT ACCEPTABLE TO THE KGARL, THE ANIMAL CAN/WILL BE REMOVED FROM FOSTER HOME AND RETURNED TO THE KING GEORGE ANIMAL RESCUE LEAGUE.

THE FOSTER HOME RELEASES KGARL FROM ANY AND ALL LIABILITY ARISING OUT OF OR IN ANY WAY CONNECTED WITH THE FOSTERING OF A KGARL PET

I CERTIFY THAT I/WE HAVE NEVER BEEN CONVICTED OF ANIMAL CRUELTY, NEGLECT, OR ABANDONMENT.

I, THE UNDERSIGNED, AFFIRM THAT THE INFORMATION CONTAINED IN THIS APPLICATION AND IN ANY ATTACHMENTS TO THIS DOCUMENT ARE BOTH CORRECT AND COMPLETE TO THE BEST OF MY KNOWLEDGE.
